Definition
Apolipoprotein A1 is the primary structural protein on HDL particles. It is a count of the return-path particles rather than the cholesterol those particles carry.
Why it matters
Read next to ApoB, it shows the balance between atherogenic particles and HDL particles. SiPhox and many clinic panels report both so the ApoB/ApoA1 ratio has a measured denominator.
When it reads low
A lower ApoA1 often travels with low HDL and an insulin-resistance pattern. It is context for the lipid panel, not a stand-alone diagnosis.
When it reads high
A higher ApoA1 usually means more HDL particles in circulation. Very high values still do not override an elevated ApoB or Lp(a).
Questions for a clinician
01
How should ApoA1 be read next to my HDL and ApoB?
02
Does the ApoB/ApoA1 ratio change anything ApoB alone does not?
03
Is this a number you would track on the next draw?
