These terms cover the public website and pilot-interest list while laboratory ordering remains closed.

Pilot-stage website

The BaselineDX website currently describes a planned Utah Valley clinic pilot. Checkout remains disabled until the laboratory, collection device, panel menu, operating procedures and applicable agreements are approved. Joining the pilot list does not create a patient relationship, guarantee eligibility or reserve a testing kit.

Not medical advice or emergency care

Website content and sample dashboard data are educational demonstrations and are not diagnosis, treatment or individualized medical advice. BaselineDX is not an emergency service. Call 911 or seek appropriate emergency care for urgent symptoms.

Future orders and clinical services

If ordering opens, the final price, included analytes, collection method, laboratory, turnaround, cancellation terms and replacement policy will be shown before purchase. Additional clinic, laboratory, consent and privacy terms may apply. The laboratory report and treating clinician—not the marketing website—are the source of clinical interpretation.

Acceptable use

You may use the site for lawful personal evaluation of the pilot. You may not attempt to disrupt the service, bypass security, scrape restricted data, impersonate another person or submit another person’s information without authority.

Intellectual property

The BaselineDX name, site design, copy, graphics and software are owned by or licensed to BaselineDX and may not be reproduced commercially without written permission. Third-party names and marks belong to their respective owners.

Disclaimers and limitation

The pilot-stage website is provided on an as-available basis. To the fullest extent permitted by law, BaselineDX disclaims implied warranties relating to the public website and is not liable for indirect, incidental or consequential damages arising from its use. Rights that cannot lawfully be limited remain unaffected.

Utah law and changes

These website terms are governed by Utah law, without regard to conflict-of-law rules. We may update the terms as the pilot becomes operational; the effective date identifies the current version.
