This notice covers the public BaselineDX website and Utah Valley pilot-interest form. It does not replace a clinic or laboratory privacy notice that may apply if you later order testing or receive clinical services.

Before ordering opens, the pilot-interest form collects only your first name, email address, panel interest and collection preference. Do not submit symptoms, diagnoses, medications, test results or other medical information through the website or ordinary email.

We may also receive ordinary technical information such as IP address, browser type and request logs used to secure and operate the site.

We use pilot-interest information to respond to you, plan pilot capacity and notify you when validated ordering details are available. We use technical information to prevent abuse, troubleshoot the service and protect the website.

BaselineDX does not sell pilot-interest information. We may use service providers that help host the website and deliver operational email, subject to their applicable terms and safeguards.

The public interest form is not a patient portal and is not designed to receive protected health information. If testing opens, clinical intake, laboratory orders and results will use separately approved systems and notices. Ordinary email will not be used to send biomarker results or diagnoses.

We retain pilot-interest information only as long as reasonably needed to operate the launch list, meet legal obligations and resolve disputes. You may ask to update or remove your interest information by contacting partners@baselinedx.com.

No system can guarantee absolute security. BaselineDX uses reasonable administrative and technical measures appropriate to the limited information collected at this stage.

We may update this notice as the pilot, vendors and data flows are finalized. Material changes will be reflected by a new effective date, and additional consent or clinical notices will be presented when required.
