What Osmolality measures

Serum osmolality measures how concentrated the blood is. In CIRS protocols it is ordered with ADH so the two can be read as a pair.

Why it matters

How clinicians usually read it

The pair is the point. A mailed chemistry sodium is not a substitute for a formal osmolality and a frozen ADH.

When it reads low

A lower value can mean

A lower osmolality means a more dilute serum. It is interpreted with ADH, sodium, and symptoms — not as a stand-alone target.

When it reads high

A higher value can mean

A higher osmolality means a more concentrated serum. Dehydration, a long fast, or an ADH mismatch can all live here.
