Total iron-binding capacity measures how much iron the blood's transferrin could carry if fully loaded. It reflects how much transferrin the liver is producing.
Why it matters
It moves opposite to iron stores: the body raises capacity when stores run low and lowers it in overload or inflammation, making it a directional clue.
When it reads low
A lower TIBC can accompany iron overload, inflammation, or lower protein production. It shifts the read of the other iron markers.
When it reads high
A higher TIBC is the classic iron-deficiency response — the body building extra carrying capacity while stores are low.
