Total testosterone is the combined bound and unbound testosterone in blood. Most is carried on SHBG and albumin; only a small fraction is immediately available to tissues.
Why it matters
It is the standard first androgen number. Morning collection, sleep, illness, and binding proteins all move it, so one value is a starting point, not a verdict.
When it reads low
A lower total can accompany sleep loss, illness, caloric deficit, pituitary or testicular issues, or normal aging. Symptoms and repeat morning testing matter.
When it reads high
A higher total can be physiologic, supplement-related, or — rarely — a signal to look for another endocrine cause. Exogenous androgens change the interpretation entirely.
