What VLDL measures
VLDL cholesterol is usually estimated as triglycerides divided by five when the draw is fasting and triglycerides are not extreme. It is derived, not a separate laboratory assay.

VLDL cholesterol is usually estimated as triglycerides divided by five when the draw is fasting and triglycerides are not extreme. It is derived, not a separate laboratory assay.
Why it matters
It translates a high triglyceride number into the particle class that carries most of that fat. ApoB still counts how many atherogenic particles are present.
When it reads low
A lower estimated VLDL usually means quieter triglyceride traffic. Fasting status still has to be true for the estimate to hold.
When it reads high
A higher estimated VLDL tracks with high triglycerides and often with insulin resistance. Markedly high triglycerides make the estimate less reliable.
